Before any meeting with a supervisor, it is always best to be prepared. It is likely that your supervisor serves as a supervisor to many other employees or students. Preparing for your evaluation will ensure that you have all the necessary evidence to demonstrate to the field supervisor that you have made progress and/or achieved the objectives set out in your learning plan. It will also provide your field supervisor with evidence that can be used to assess your performance on other evaluation criteria. Be sure that your supervisor has received a copy of both the Supervisor Evaluation sheet and your self-evaluation prior to your meeting.

Supervisor Evaluation (Google Forms)
Be sure to email the Google Forms to your field supervisor prior to meeting with him/her AND request confirmation they have received it. If you do not get confirmation from your field supervisor be sure to PRINT out the supervisor evaluation form in Google docs (required resource #2) for them.
